Mandatory Documents
Students will not be allowed to start at JESS until the correct documentation is received.
Medical Declaration (see form attached below)
All parents are required to complete a JESS Medical declaration. This requests information about any medical conditions we should be aware of, their immunisation history and also gives parental consent for emergency treatment.
Off-Site Permission Form (see form attached below)
Sixth Form students enjoy the privilege of being able to leave the school premises and visit the Arabian Ranches Shopping Centre beside JESS provided the school has the permission of the parents. If you would like your son/daughter to also enjoy this privilege please read the conditions outlined in the form below and sign and return the tear-off slip attached.
Academic Honesty Contract (see form attached below)
All Sixth Form students and parents must read the Academic Honesty Contract and return the tear-off slip signed by both the student and the parent.
